The paramedic displays with an evil grin and straps you naked and spread eagle to something cold and hard. He takes out a screwdriver with feathers attached to the end.

"Let's begin" he says.

He proceeds to torture you for hours. You feel the strain it is having on your heart. You suddenly have a heart attack. The paramedic, seeing this, takes out his defibrillator panels and shocks you.

You JOLT awake. You are in the exact same ambulance, with the exact same paramedic, but now looking at you with a gentle smile.
