You really want to help, but your life is on the line. You walk by the old lady and hear her say something that sounded like, 'Repulse,' but you can't be certain. It turns out the old lady was a witch and she cursed you by being repulsive to all women. You and your wife divorce soon after and you die alone and miserable, without ever having sex again. The curse never lifts throughout your long, miserable life. Perhaps you should have been kind and helped the old lady after all.
