The Xtreme Championship Wrestling (XCW) World Heavyweight Championship was a professional wrestling world heavyweight championship in Xtreme Championship Wrestling (XCW) and International Wrestling Entertainment (IWE). It was the original world title of the Extreme Championship Wrestling promotion, later used in IWE as the world title of the XCW brand and one of three in IWE, complementing the WWE Championship and World Heavyweight Championship. The championship also briefly appeared as the only world title featured on the Raw brand in 2008. It was established under XCW in 1994 but was originally introduced in 1992 by the precursor to XCW, Eastern Championship Wrestling.

Overall, there have been 49 XCW World Heavyweight Championship reigns shared among 32 individuals. The inaugural champion was Jimmy Snuka, who won the title by defeating Salvatore Bellomo in April 1992. The Sandman holds the most reigns as champion, with five. Shane Douglas, in his fourth reign, had the longest reign in the title's history which lasted 406 days, while Russell Dalton, in his first (and only, due to his defeating of Khristian on the television finale of XCW
